3. Effectieve implementatie van verbetering in de zorg: een systematische aanpak

Samenvatting

In dit hoofdstuk wordt een planmatige aanpak voor implementatie gepresenteerd. Planmatig betekent hier niet dat een definitief plan wordt gemaakt waarvan niet meer wordt afgeweken. Integendeel, er is sprake van een ‘iteratief’ of ‘incrementeel’ proces, waarin steeds wordt geleerd van reeds gezette stappen en waarin de aanpak steeds wordt bijgesteld als dat nodig is. Het ontwikkelen en vaststellen van een concreet, haalbaar voorstel voor gewenste verandering in de praktijk is de eerste stap in de aanpak. Alvorens over te gaan tot activiteiten gericht op implementatie van de innovatie is het nodig de feitelijke zorgverlening goed te kennen: welke zorg wordt nu geleverd, is deze conform de voorgestelde werkwijze en waar liggen de belangrijkste afwijkingen? Het analyseren van de context waarbinnen verandering van routines plaatsvindt, van kenmerken van de doelgroep en van bevorderende en belemmerende factoren ten aanzien van de voorgestelde verandering vormt de volgende stap in het model. Stappen in een veranderingsproces zijn: oriëntatie, inzicht, acceptatie, verandering en behoud. Gekoppeld aan gevonden factoren en andere informatie wordt samen met de doelgroep een kosteneffectieve mix van maatregelen, methoden en strategieën ten behoeve van de implementatie ontwikkeld en eventueel getest. Bij de feitelijke uitvoering wordt vastgesteld op welk niveau welke interventies en maatregelen het best kunnen worden gepland. Het is cruciaal dat de verbeteringen in de zorgverlening zo goed mogelijk worden ingebouwd in de vaste routines en worden verankerd in de organisatie en dat terugval wordt voorkomen. Een laatste stap in de implementatie van innovaties in de zorg is de evaluatie van de bereikte resultaten: zijn de beoogde doelen behaald? Stel een team samen met voldoende expertise en veel motivatie om het project te coördineren en te stimuleren.
