Effect van tekort en suppletie van groeihormoon en insuline-achtige groeifactor-I op hersenfuncties

Samenvatting

Groeihormoon (GH) is één van de hormonen die worden afgegeven door de hypofysevoorkwab. De secretie van GH staat onder invloed van de afgifte van growth hormone relasing hormone(GHRH) uit de hypothalamus. GH wordt pulsatiel afgegeven door de hypofyse en stimuleert onder andere de lever om insuline like growth factor 1(insuline-achtige groeifactor-I; IgF-I) te maken, wat weer invloed heeft op de rest van het lichaam (o.a. op de botten, spieren en hersenen). Bij het behandelen en bestuderen van patiënten met een GH-deficiëntie (GHD) is het belangrijk onderscheid te maken tussen GHD ontstaan in de kindertijd (childhood-onset[CO]-GHD) of op latere leeftijd (adult-onset[AO]-GHD). Een ander belangrijk onderscheid is dat tussen geïsoleerde GHD (IGHD) en multiple pituitary hormone deficiency(MPHD). Bij IGHD is er slechts een tekort aan GH-secretie, terwijl bij MPHD naast een tekort aan GH-secretie ook een tekort is aan de secretie van andere hypofysehormonen (ACTH, TSH, gonadotropines).
