12. Eetstoornissen en vroegkinderlijk trauma en PTSS

Samenvatting

Veel cliënten met een eetstoornis hebben in hun verleden een of andere vorm van vroegkinderlijk trauma meegemaakt. Sommigen ontwikkelen PTSS, andere cPTSS of krijgen hechtingsproblemen. Alle vormen van traumatisering kunnen een predisponerende factor zijn in de etiologie van eetstoornissen en de behandeluitkomst negatief beïnvloeden. Anders dan bij niet-getraumatiseerde cliënten heeft de eetstoornis ook een functie in de emotieregulatie. Om een goede behandelindicatie te stellen, is het belangrijk om na te gaan of er wel of geen daadwerkelijke PTSS-klachten aanwezig zijn. Geadviseerd wordt bij evidente PTSS-klachten al in een vroeg stadium de behandeling op het trauma te richten en pas later met de daadwerkelijke eetstoornisbehandeling te starten. Is er geen sprake van PTSS, kan direct met een reguliere eetstoornisbehandeling begonnen worden. In geval van cPTSS of hechtingsproblematiek die een eetstoornisbehandeling in de weg staan, kan overwogen worden om een behandeling te starten die zich daarop richt.
