11. Eetstoornissen en angst- of stemmingsstoornissen

Samenvatting

Angst- en stemmingsstoornissen behoren tot de veelvoorkomende comorbide klachten bij eetstoornissen. Angstklachten gaan vaak aan de eetstoornis vooraf, terwijl stemmingsklachten zowel primair als ook secundair aan de eetstoornis kunnen zijn. Er zijn aanwijzingen dat angst- en stemmingsklachten een gemeenschappelijke basis hebben met eetstoornissen. Angst voor controleverlies en problemen met de emotieregulatie spelen daarbij een rol. Angst- en stemmingsklachten kunnen een eetstoornisbehandeling bemoeilijken. Ze hebben een negatief resultaat op de uitkomst en verhogen de kans op drop-out. Bij primaire en ernstige depressies is het aan te raden in eerste instantie te focussen op de stemmingsklachten, eventueel kan dat medicamenteus. Bij secundaire depressie kan gewoon gestart worden met een reguliere eetstoornisbehandeling. Angstklachten kunnen in de meeste gevallen gewoon na de eetstoornis behandeld worden.
