Een pijnlijke smalle pols na val van paard: traumatische volaire dislocatie van het distale radio-ulnaire gewricht

Samenvatting

Traumatische dislocatie van het distale radio-ulnaire gewricht is een zeldzame aandoening die geassocieerd is met letsels van de primaire en secundaire stabilisatoren van het radio-ulnaire gewricht. Wij presenteren een casus van een 21-jarige vrouw met een volaire dislocatie van de distale ulna, die initieel gemist werd. Gesloten repositie was niet succesvol en er werd besloten tot open repositie. Tijdens de operatie werd het triangulair fibrocartilagineuze complex (TFCC) gere-insereerd in de fovea van de ulna. De geluxeerde pees van de m. extensor carpi ulnaris werd gestabiliseerd met een sling van het retinaculum extensorum manus. De pols werd gedurende zes weken gefixeerd met twee transossale K-draden en een onderarmgips. Na zes maanden had de patiënt een volledige polsfunctie.
