Samenvatting

Dyspareunie is een hinderlijke, schaamtevolle klacht waarbij het niet eenvoudig is een diagnose te stellen. Het is vaak een multifactoriële aandoening. De meeste patiënten zijn jonge vrouwen. Ernstige oorzaken, waar de dokter alert op moet zijn bij diepe dyspareunie, zijn tumoren in het kleine bekken en soa’s. De meest gestelde diagnose bij oppervlakkige dyspareunie is bij jonge vrouwen provoked vestibulodynie (PVD) en bij postmenopauzale vrouwen urogenitale atrofie. Andere mogelijk oorzaken bij de vrouw zijn: vulvovaginitis, bartholinitis, anatomische, traumatische en gynaecologische afwijkingen, opwindingsstoornis, vaginisme, bekkenbodemoveractiviteit en het geven van borstvoeding. Bij mannen kan het gaan om balanitis, phimosis, een te kort frenulum, de ziekte van Peyronie of het bekkenpijnsyndroom. De oorzaken van dyspareunie zijn meestal niet levensbedreigend. Dyspareunie is wel belastend voor de patiënt, omdat het een negatieve invloed heeft op het welbevinden en op de relatie met de partner. Een zorgvuldige anamnese waarbij wordt gevraagd naar het eerste moment waarop de pijn ontstond, het beloop en de precieze locatie van de pijn levert voor het stellen van de diagnose belangrijke informatie op.
