Samenvatting

De behandeling en zorg voor patiënten met hartfalen zijn vaak georganiseerd in zogeheten diseasemanagementprogramma’s, ook wel hartfalenzorgprogramma’s genoemd. Er bestaan verschillende soorten hartfalenprogramma’s, zoals een hartfalenpoli of transmurale zorg. Bij de professionele zorgverlening zijn zorgverleners betrokken uit diverse disciplines. Multidisciplinaire hartfalenzorg kan leiden tot betere uitkomsten bij de patiënt (bijvoorbeeld betere zelfzorg, minder heropnames) en voor de gezondheidszorg (bijvoorbeeld minder kosten). Goede onderlinge communicatie en samenwerking tussen de betrokken zorgverleners is hierbij belangrijk, evenals de mogelijkheid tot een laagdrempelig contact met de patiënten.
In internationale en nationale richtlijnen wordt een aantal belangrijke onderdelen van een hartfalenzorgprogramma benoemd die optimale diagnostiek, behandeling en nazorg omvatten. Zorg voor hartfalen is ook maatwerk en dient steeds te worden aangepast aan de individuele patiënt. In de behandeling is een belangrijke rol weggelegd voor de patiënt zelf, van wie een hoge mate van zelfzorg en therapietrouw wordt verwacht. Niet alle patiënten zijn even goed in staat deze rol op zich te nemen en daarom is een goed sociaal en professioneel netwerk rond de patiënt belangrijk.
