Diagnostiek en classificatie van letsels van de thoracolumbale wervelkolom

Samenvatting

Thoracolumbale wervelfracturen worden meestal veroorzaakt door hoogenergetisch trauma zoals verkeers- en industrieongevallen. De afgelopen jaren zijn de kennis over de biomechanica van de wervelkolom en de diagnostische mogelijkheden sterk verbeterd. Diagnostiek vindt vooral plaats door middel van conventioneel röntgenonderzoek, aangevuld met computertomografie (CT) en eventueel MRI. De neurologische status wordt geclassificeerd volgens Frankel. Voor de fractuur zijn diverse classificatiesystemen ontwikkeld. In dit artikel wordt de classificatie volgens Magerl gehanteerd, waarbij zowel anatomische aspecten (3-pijlermodel) als het letselmechanisme een rol spelen. Er worden drie fractuurtypen onderscheiden met een toenemende ernst: compressieletsels (A), distractieletsels (B) en rotatieletsels (C). De instabiliteit neemt toe met de ernst van het letsel en kan naar Vaccaro nader worden geclassificeerd op basis van de integriteit van het posterieure ligamentencomplex en de neurologische status van de patiënt.
