Determinanten van ziekte en gezondheid op hoge leeftijd:

Nieuwe inzichten uit de Leidse verouderingsstudies

Samenvatting

Het klinisch wetenschappelijk onderzoek van de afdeling Ouderengeneeskunde van het Leids Universitair Medisch Centrum is gericht op de ontrafeling van de oorzaken van ziekte en gezondheid op hoge leeftijd. Wij onderzoeken onder meer de genetische mechanismen van langlevendheid door het vergelijken van kinderen van langlevende ouders met hun partners. De gemiddeld 60-jarige kinderen van de langlevende ouders hebben een lagere prevalentie van verschillende cardiometabole ziekten, waaronder hartinfarct, suikerziekte en hoge bloeddruk. Tevens vertoonden de kinderen van de langlevende ouders een gunstiger risicoprofiel, met lagere waarden voor glucose, insuline, triglyceriden en schildklierhormoon, en een betere insuline sensitiviteit. Daarnaast hebben wij de afgelopen jaren aangetoond dat traditionele cardiovasculaire risicofactoren, zoals een verhoogd cholesterol en hypertensie, niet automatisch toe te passen zijn op de oudste ouderen. Op zeer hoge leeftijd moet men dus rekening houden met het feit dat de risicoprofielen voor ziekte anders liggen dan bij een jongere populatie. Daarom dienen preventieve en therapeutische ingrepen onder strikte controle worden ingezet. De keuze voor de (medicamenteuze) behandelingen moet zijn gebaseerd op de ‘best available evidence’. In afwezigheid van gerandomiseerde studies is dit vooralsnog de wetenschappelijk gefundeerde kennis van de pathofysiologie van ziekte en gezondheid op hoge leeftijd.
