2. De schoudergordel in evenwicht

Samenvatting

De bouw en functie van een lichaamsdeel hangen sterk met elkaar samen. Dat geldt ook voor het schoudergewricht. De grote mobiliteit van de schoudergordel wordt mogelijk gemaakt door de scapula en de clavicula, in combinatie met de rotaties in het glenohumerale gewricht. De scapula vormt de beweegbare én stabiele basis van waaruit glenohumerale rotatie plaatsvindt doordat de scapula altijd op drie punten contact houdt met de romp: via de mediale rand van de scapula en via het acromioclaviculaire gewricht. In het glenohumerale gewricht zorgt de compressiekracht, gevormd door externe krachten en spierkrachten, ervoor dat kop en kom bij elkaar blijven. De spieren van de schoudergordel spelen een essentiële rol in de stabilisatie van het gewricht, dat zijn de scapulothoracale spieren, de scapulohumerale spieren en de spieren die de humerus aandrijven. Voor de stabiliteit van de schoudergordel zijn snelle reflexen belangrijk om verstoringen op te vangen.
De structuur van het gewricht heeft niet alleen invloed op de functie, ook het omgekeerde is het geval. Het skelet past zich aan aan eenzijdig gebruik, vooral wanneer dat in de groei plaatsvindt. Schijnbaar dyskinetische bewegingspatronen kunnen functionele aanpassingen aan een veranderde structuur zijn. Daarom moet bij diagnostiek en behandeling van schouderaandoeningen goed worden gekeken naar de configuratie van het skelet. Dit betekent dat de gebruikelijke links-rechtsvergelijking niet altijd voldoende is en en dat er ook rekening gehouden moet worden met een verschil in belasting tussen beide zijden.
