Doel

Consulten voor alledaagse klachten vormen een aanslag op de werklast van de huisarts. Specifieke communicatiestrategieen, zoals actief luisteren en het wekken van positieve uitkomstverwachtingen, dragen mogelijk bij aan zelfzorg bij de patiënt.
Bij dit onderzoek zijn we nagegaan hoe vaak huisartsen dergelijke strategieën hanteren en hoe deze samenhangen met ervaren gezondheid, consultfrequentie en medicatietherapietrouw.

Methode

In 524 op video opgenomen consulten met patiënten met alledaagse klachten observeerden we de volgende strategieen: actief luisteren, positieve communicatie, patiëntgerichtheid, het (wetenschappelijk) onderbouwen van advies en het voortbouwen op eerdere behandelervaringen. De patiënten vulden vragenlijsten in. Informatie over consultfrequentie was afkomstig van de doorlopende patiëntenregistratie. We analyseerden de verkregen gegevens aan de hand van multivariate regressieanalyse.

Resultaten

Het uiten van een gunstige prognose blijkt gerelateerd aan een betere gezondheid, maar alleen als de huisarts gelijktijdig een verklaring voor de oorzaak van de klachten geeft. Zo’n verklaring is ook gerelateerd aan een betere gezondheidstoestand bij de patiënt, behalve wanneer hij zich bij binnenkomst somber voelt. Actief luisteren en betrokkenheid bij de besluitvorming blijken bij deze subgroep samen te hangen met grotere therapietrouw.

Conclusie

Patiënten met alledaagse klachten lijken gebaat bij een huisarts die duidelijk én optimistisch is over de aard en de oorzaak van de klachten, behalve wanneer zij zich somber voelen. Dan lijkt vooral een luisterend oor te werken. Huisartsen doen er dus goed aan zich te vergewissen van de emotionele toestand van de patiënt.
