De invloed van multimorbiditeit op het cognitief functioneren

Doel Multimorbiditeit komt veel voor in de huisartsenpraktijk. Men associeert specifieke ziekten vaak met cognitieve achteruitgang, maar er is weinig bekend over de invloed van multimorbiditeit op het cognitief functioneren.

Methode We selecteerden personen tussen de 24 en 81 jaar (n = 1763) uit het RegistratieNet Huisartspraktijken (RNH) voor deelname aan de Maastricht Aging Study (MAAS), een longitudinaal onderzoek dat de determinanten van cognitieve veroudering in kaart moet brengen. We onderzochten de relatie tussen 96 chronische ziekten, gegroepeerd in 23 clusters, en cognitief functioneren op baseline, na 6 en na 12 jaar. We hebben het cognitief functioneren in twee domeinen gemeten: verbaal geheugen en informatieverwerkingssnelheid.

Resultaten Multimorbiditeit was aanwezig bij 55% van de onderzoekspopulatie. Verscheidene ziekteclusters hadden een negatief effect op het cognitief functioneren tijdens de 12 jaar durende follow-up. Een maligniteit in combinatie met een aandoening aan het bewegingsapparaat vertoonde een negatieve invloed op het cognitief functioneren.

Conclusie De resultaten wijzen erop dat een groot aantal ziekten negatief geassocieerd is met het cognitief functioneren. Deze effecten blijken echter over een periode van twaalf jaar relatief klein.
