16. De impact van traumatisering in de kindertijd en PTSS bij mensen met psychotische en andere ernstige psychiatrische aandoeningen

Samenvatting

Traumatisering in de kindertijd is een van de oorzaken van psychose. Omdat posttraumatische stressstoornis als gevolg van ernstige traumatisering een blijvend karakter heeft, leidt dit bij vele EPA-patiënten tot grote lijdensdruk en meerdere psychotische episoden. Ook leidt traumatisering in de jeugd tot achterstanden bij de neuro-ontwikkeling die vergelijkbaar zijn met die bij schizofrenie. In de afgelopen decennia is duidelijk geworden dat posttraumatische stressstoornis bij psychotische stoornissen goed en veilig is te behandelen en dat ook psychose en depressie als gevolg van de behandeling verder afnemen. In dit opzicht is de behandeling met Exposure Therapy en/of Eye Movement Desensitisation and Reprocessing (EMDR) gewenst. Na het bestrijden van de stoornis als gevolg van de trauma’s is verder soms ook behandeling van dissociatieve en depersonalisatie-ervaringen en het verbeteren van emotieregulatie nodig om de lijdensdruk te verminderen en brengt dit tevens sociaal, maatschappelijk en persoonlijk herstel dichterbij.
