De impact van comorbiditeit op complicaties na (partiële) tumornefrectomie

DE IMPACT VAN COMORBIDITEIT OP COMPLICATIES NA (PARTIËLE) TUMORNEFRECTOMIE

Samenvatting

Er is momenteel geen consensus over het rapporteren van complicaties na (partiële) tumornefrectomie. Niet eerder is objectief onderzocht of comorbiditeit invloed heeft op complicaties na tumornefrectomie met behulp van een systematisch scoringssysteem.
Data van 222 patiënten die een (partiële) tumornefrectomie ondergingen in het Universitair Medisch Centrum Utrecht tussen 1999 en 2008 werden retrospectief verzameld. Post operatieve complicaties werden gescoord volgens de gemo dificeerde Clavien- classificatie (CCSC) en comorbiditeit volgens de Charlson Comorbiditeit Index (CCI). Bij 53% van de patiënten was sprake van preoperatieve comorbiditeit. Er werden 88 complicaties geregistreerd bij 76 (34,2%) patiënten, waarvan 7,2% graad 1, 14,9% graad 2, 6,3% graad 3, 2,2% graad 4 en 3,6% graad 5. Patiënten met ernstige comorbiditeit (> 2 CCI) hadden meer kans op ernstige complicaties (CCSC 3-5) (p = 0,017). Daarnaast nam de gemiddelde opnameduur significant toe met de ernst van de postoperatieve complicaties (p < 0,001).
Concluderend is het van belang de comorbiditeit van een patiënt te betrekken bij de preoperatieve besluitvorming en postoperatieve behandeling.
