Samenvatting

In dit hoofdstuk wordt de KNGF-richtlijn ‘Artrose heup-knie 2018’ geïmplementeerd en melding gemaakt van recent belangrijk wetenschappelijk onderzoek. De configuratie van de heupkop en -kom, het collum femoris, de bekkentypen en het intra-articulaire corpus adiposum worden besproken in relatie tot het in dit boek gepropageerde concept van de bindweefselplaten. Er worden effectieve behandelmethodes geboden voor milde vormen van het femoro-acetabulair impingementsyndroom en aspecifieke heup- en liespijn. Zoals bij de knie(artrose) is er ook bij de heup wetenschappelijke onderbouwing voor het feit dat er veel te snel wordt overgegaan tot operatieve vervanging. Met manuele therapie en graded-activity-oefentherapie kan er bewezen effectief conservatief worden behandeld en het preoperatieve traject worden verlengd met respect voor de contextuele factoren, zoals (ernstige) comorbiditeit. Belangrijke klinimetrische hulpmiddelen als de Harris Hip Score, Algofunctional Index en HOOS komen uitgebreid aan bod. Voorbeelden van specifieke heuppijn worden in dit hoofdstuk gekoppeld aan diagnoses als congenitale heupdysplasie, coxa valga/coxa vara, epifysiolysis capitis femoris, morbus Perthes, heupluxatie (dashboardtrauma), gluteale claudicatie en uiteraard heupartrose. Verder komen de overige rode vlaggen ter sprake, zoals heupfracturen, het ‘sign of the buttock’ (bilteken), het teken van Lasègue en het lumboradiculairsyndroom. Er is ook een handige ‘patronenmatrix’. Maar liefst 36 onderzoeks- en behandeltechnieken, waaronder evidence-based HVT-manipulaties, worden uitgebreid beschreven en op video gedemonstreerd.

BSE

De bloedbezinkingssnelheid (in mm/uur) van de rode bloedcellen of erytrocyten ( = BSE) wordt gemeten door centrifugatie van onstolbaar gemaakt bloed. Normaalwaarden (aantal mm/bezinksel per uur):

– pasgeborenen (neonaten): 0–2,2 mm/uur;

– kinderen onder de 10 jaar: < 3–13 mm/uur;

– vrouwen onder de 50: < 20 mm/uur;

– vrouwen boven de 50: < 30 mm/uur;

– mannen onder de 50: < 15 mm/uur;

– mannen boven de 50: < 20 mm/uur.

osteopenie

Situatie waarbij de botmassa en botdichtheid zijn afgenomen. Door afname van botmineralen (botontkalking) neemt de kans op fracturen toe. Osteopenie is het voorstadium van osteoporose. Osteopenie gaat meestal niet gepaard met klachten.

osteoporose

Deze aandoening is een ernstiger vorm van osteopenie. Formeel wordt gesproken van osteoporose als bij een DEXA-scan (botdichtheidsmeting) een T-waarde van −2,5 (of lager) is vastgesteld. Bij osteoporose is meestal sprake van pijnklachten, bijvoorbeeld door ingezakte wervels.

Predisponerende factoren en signalen voor osteopenie/osteoporose:

– calcium- (kalk) en/of vitaminetekort;

– botbreuk na het 50^ste levensjaar;

– osteoporose in de familie;

– te lage BMI (te laag lichaamsgewicht in verhouding tot lichaamslengte);

– bedlegerigheid, onderbelasting;

– langdurig gebruik van corticosteroïden, zoals prednisolon, prednison;

– tekort aan geslachtshormonen: de oestrogeenproductie bij vrouwen stopt te vroeg, bijvoorbeeld door een vroege overgang of door chemotherapie of komt te laat op gang door op jonge leeftijd dagelijks aan topsport te doen;

– eetstoornissen;

– afwijkingen van de schildklier of bijschildklier;

– reumatoïde artritis.

prehabilitatie

Het voorbereiden op zowel de operatie als de revalidatie, bij voorkeur door een deskundige therapeut. Door uitleg en goede begeleiding wordt de patiënt gemotiveerd om het lijf in zo goed mogelijke conditie te brengen voor de operatie. Het verbeteren van de fitheid kan worden gefaciliteerd door manuele therapie in enge zin en een ‘graded activity’-programma. De trofische omstandigheden en mobiliteit van de weefsels (rond bijvoorbeeld de artrotische heup, knie of enkel) en de overige gewrichten worden regelmatig gecontroleerd. Hier geldt: ‘Better In Better Out’.

primaire techniek

Een techniek waarbij het te bewegen botstuk onderdeel uitmaakt van het te behandelen gewricht. Bijvoorbeeld: bij de primaire heuptractiemanipulatie wordt het femur omvat.

secundaire techniek

Een techniek waarbij de beweging teweeg wordt gebracht via een botstuk dat niet deelneemt aan het te behandelen gewricht. Bijvoorbeeld: bij de secundaire heuptractiemanipulatie wordt het onderbeen omvat.
