De fonologische variant van primaire progressieve afasie, een gevalsstudie

Samenvatting

Een primaire progressieve afasie (PPA) ontstaat geleidelijk en neemt langzaam toe. Elk aspect van het taalgebruik kan worden aangetast (woordvinding, benoemen, vloeiende spraak, grammatica, klankvorming en begrip). Andere cognitieve vaardigheden blijven echter behouden, zoals het geheugen voor recente gebeurtenissen, visueel-ruimtelijke en visueel-constructieve vaardigheden. Onlangs werd internationaal overeenstemming bereikt over het onderscheid tussen drie varianten van PPA: de progressieve niet-vloeiende variant (nfvPPA), de semantische variant (svPPA) en de logopenische/fonologische variant (lvPPA). Hier wordt een 79-jarige man beschreven wiens symptomencomplex overeenkomt met de logopenische/fonologische variant van primaire progressieve afasie (lvPPA). Zijn spontane spraak werd gekenmerkt door een hoog spreektempo, zonder articulatieproblemen, maar met opvallende woordvindingsmoeilijkheden en foneemsubstituties. Afbeeldingen van voorwerpen kon hij niet altijd benoemen, succes was sterk afhankelijk van de frequentie van het gezochte woord in de taal. Hij ondervond veel problemen bij het nazeggen van zinnen en meerlettergrepige woorden. Woorden met een regelmatige spellingklankrelatie las hij goed voor, maar dat gold veel minder voor woorden met een onregelmatige relatie tussen spelling en klank (oppervlakte-alexie). Hij begreep de betekenis van gesproken woorden, maar dat gold veel minder voor complexe zinsconstructies. Behoud van semantisch-conceptuele kennis werd afgeleid uit zijn normale prestatie op de visuele Semantische Associatie Test (SAT). Er was een lichte deficiëntie op de verbale SAT, die een beroep doet op kennis van woordassociaties. De cruciale stoornis werd gelokaliseerd in een verzwakt vermogen om vanuit het semantisch systeem de juiste lexicaal-fonologische codes te activeren voor spreken, benoemen, nazeggen en hardop lezen.
