29. De bovenste extremiteiten

Samenvatting

Het onderzoek van de bovenste extremiteiten bestaat uit het schouder-, elleboog- en hand-/polsonderzoek. Zorg dat de patiënt voldoende is ontkleed om de samenhang tussen de gewrichten en links-rechtsverschillen te kunnen zien. Achtereenvolgend worden inspectie, bewegingsonderzoek, palpatie en de specifieke testen uitgevoerd. Tijdens de onderzoeken is het belangrijk de patiënt te observeren (bewegingsbeperking, pijn), de romp te stabiliseren en de juiste handzetting toe te passen voor zowel de ondersteuniging van de te onderzoeken gewrichten als de juiste bewegingsuitslag. Afwijkingen bij het lichamelijk onderzoek moeten altijd geduid worden in het kader van de context (anamnese) van de patiënt: de wetenschappelijk waarde van een afwijkende geïsoleerde test is vaak gering. De betrouwbaarheid van het lichamelijk onderzoek is groter als de combinatie van anamnese, inspectie, bewegingsonderzoek, palpatie en de specifieke test bij een diagnose passen.
