Samenvatting

De bijdrage van Paul Lamers (2008) over risico’s en bijwerkingen van complexe traumabehandeling aan de rubriek ‘Forum’ vraagt om een repliek. De behandeling van mensen met complexe traumagerelateerde stoornissen en een geschiedenis van onder meer vroegkinderlijke chronische traumatisering is, zoals Lamers stelt, inderdaad complex, vergt veel van patiënten en therapeuten, en is niet van risico’s ontbloot. Zo komen tal van patiënten met een dergelijke achtergrond in behandeling met niet alleen specifieke symptomen van een posttraumatische stressstoornis (PTSS) maar, naast persoonlijksstoornissen en hardnekkige hechtingsstoornissen, ook met ernstige lichamelijke klachten, forse depressieve klachten, zelfbeschadigend gedrag, suïcidale gedachten en handelingen, psychotische episodes, middelenafhankelijkheid en complexe dissociatieve stoornissen: verschijnselen die alle aanleiding zijn geweest om deze patiënten uit te sluiten van effectonderzoek bij PTSS (Spinazzola, Blaustein & Van der Kolk, 2005).

In Nederland heeft zich in dit opzicht een essentiële ontwikkeling voorgedaan. In 2007 is het Landelijk centrum voor vroegkinderlijke chronische traumatisering (LCVT) van start gegaan. Hieraan zijn tien regionale instellingen met een afdeling voor hetzij volwassenen hetzij kinderen met een dergelijke geschiedenis verbonden (zie www.lcvt.nl). Het centrum houdt zich onder meer bezig met de ontwikkeling van richtlijnen voor diagnostiek en behandeling en met deskundigheidsbevordering in de vorm van training, consultaties en supervisies. Ongetwijfeld zal daar ook uitgebreid aandacht besteed worden aan de problematiek waarop Lamers zich in zijn bijdrage heeft gericht. Vooralsnog concentreert het LCVT zich op patiënten met complexe PTSS, persoonlijkheidsstoornissen met een hoge mate van dissociatie en complexe dissociatieve stoornissen. Maar vroegkinderlijke chronische traumatisering kan ook een belangrijke etiologische factor zijn bij andere stoornissen, zoals verslavingen (zie bijvoorbeeld Langeland, Draijer & Van den Brink, 2004) en psychotische stoornissen (Moskowitz, Schäfer & Dorahy, 2009). Ook hierbij zal de ontwikkeling van passende behandeling en bijbehorende richtlijnen ter hand genomen moeten worden.
