Copingstijlen van patiënten met hersenletsel en familieleden in een revalidatiesetting

Resumen

Niet aangeboren hersenletsel is een belangrijk gezondheidsprobleem, waar alleen al in Nederland ieder jaar 1 à 2 per 1000 nieuwe gevallen bijkomen. Niet alleen de levens van de patiënten zelf, maar ook die van de familieleden staan op zijn kop, en ze worden gedwongen aanpassingen te maken aan de nieuwe realiteit. Er is weinig bekend over de factoren die een succesvolle aanpassing voorspellen. De manier waarop iemand omgaat met alledaagse problemen (coping) lijkt een belangrijke rol te spelen. In dit artikel beschrijven we in het kort wat er bekend is over coping na hersenletsel bij zowel patiënten als familieleden. Vervolgens lichten we het onderzoek toe dat we hebben verricht in een revalidatiepopulatie met hersenletsel (n=136). Uit onze resultaten bleek dat er geen associatie bestaat tussen de coping stijl van een patiënt en diens cognitief functieprofiel na het letsel, dat patiënten met een hoger opleidingsniveau vaker gebruik maakten van actieve (gunstige) coping stijlen, en dat patiënten in een latere fase na het letsel vaker gebruik maakten van passieve (ongunstige) coping stijlen. Verder bleken passieve stijlen ongunstig te zijn voor psychosociaal functioneren bij zowel patiënten als mantelzorgers. Ook vonden we dat passieve stijlen toenamen en actieve stijlen afnamen gedurende de revalidatie. Tot slot bespreken we implicaties voor verder onderzoek en de klinische praktijk.
