Continue diepe sedatie bij patiënten in de laatste levensfase

Doel We beschrijven de praktijk van continue diepe sedatie voorafgaand aan het overlijden in Nederland in 2005 en vergelijken deze met die van 2001.

Opzet Beschrijvend onderzoek en longitudinale vergelijking.

Methode We selecteerden 6860 sterfgevallen in 2005 en 5617 sterfgevallen in 2001 en stuurden een vragenlijst naar de artsen die het doodsoorzakenformulier hadden ingevuld voor overledenen uit deze steekproeven. De respons was 78% voor de selectie uit 2005 en 74% voor 2001.

Resultaten Toepassing van continue sedatie is toegenomen van 5,6% (95%-BI 5,0%-6,2%) van alle sterfgevallen in 2001 tot 7,1% (95%-BI 6,5%-7,6%) van alle sterfgevallen in 2005. We zien de toename vooral bij patiënten onder behandeling van huisartsen en bij patiënten met kanker. Bij 83% van de patiënten gebruikte de arts benzodiazepinen voor continue diepe sedatie, en bij 94% van de patiënten duurde de sedatie minder dan een week. Een deel van de patiënten (9%) had eerder een uitdrukkelijk euthanasieverzoek gedaan dat niet werd ingewilligd; 9% van de artsen raadpleegde een palliatieve expert in de laatste maand voor het overlijden.

Conclusie Continue diepe sedatie werd in de onderzochte periode vaker toegepast, waarbij er relatief weinig overleg was met palliatieve zorgexperts. Dit suggereert dat artsen de toepassing van continue diepe sedatie steeds meer zien als normaal medisch handelen.
