Congenitale afwijkingen aan de urethra; hypospadie/epispadie

Samenvatting

Aangeboren afwijkingen van de penis en urethra komen veelvuldig voor. Met name hypospadie wordt bij 1 : 200 pasgeboren jongens gevonden. De behandeling is mede afhankelijk van de ernst van de aandoening en wordt meestal op de leeftijd van een jaar uitgevoerd. De meeste hypospadiecorrecties worden in dagbehandeling of korte klinische opname uitgevoerd. In centra met voldoende ervaring zullen de resultaten in de regel goed zijn. Langdurige controle is echter noodzakelijk, omdat sommige complicaties zich pas op oudere leeftijd manifesteren. Een epispadie kan zowel bij jongens als meisjes voorkomen en is als geïsoleerde aandoening zeldzaam: de behandeling vindt daarom alleen plaats in daartoe toegeruste centra. Bij een aantal van de kinderen is ook het continentiemechanisme niet goed aangelegd en zal de behandeling alleen al om die reden veel complexer zijn dan het reconstrueren van een hypospadie.
