12. Compressieneuropathie van de nervi ulnaris en peroneus

Inleiding

Na het carpale-tunnelsyndroom zijn de nervus ulnarisneuropathie ter hoogte van de elleboog en de nervus peroneusneuropathie ter hoogte van het fibulakopje de meest voorkomende focale neuropathieën. Deze drie samen vormen ongeveer 90% van alle focale neuropathieën. De oorzaak is meestal acute of chronische compressie. Dit kan lokale demyelinisatie veroorzaken, waarbij de dikste zenuwvezels het eerst en het sterkst zijn aangedaan. Secundair kan lokale demyelinisatie leiden tot axonaal verval. Het routine zenuwgeleidingsonderzoek is zeer geschikt om een compressieneuropathie te onderzoeken, omdat dit vooral informatie geeft over de snelheid van de dikste en dus de snelst geleidende motorische en sensibele zenuwvezels, en over het gedrag van de motorische en sensibele responsen (respectievelijk CMAP en SNAP).
