Complex trauma en complexe PTSS

Wat is het en wie heeft het?

Samenvatting

Er bestaat onder clinici veel onduidelijkheid over wat de concepten complex trauma en complexe posttraumatische stressstoornis (PTSS) behelzen. Dit heeft ongewenste consequenties voor de behandeling, omdat de behandelrichtlijnen voor complexe PTSS afwijken van die voor PTSS. In dit artikel wordt verhelderd wat er onder complex trauma en complexe PTSS verstaan wordt, om zo bij te dragen aan een meer gefundeerd gebruik van deze termen. Complex trauma wordt gekenmerkt door elementen als herhaling, langdurigheid, interpersoonlijke context en verstoring van ontwikkelingsfasen. Complexe PTSS is niet als afgebakende classificatie opgenomen in het ‘Diagnostic and statistical manual of mental disorders’ (DSM) en kan niet als diagnose zonder PTSS gesteld worden. Het is van belang complex trauma en complexe PTSS te specificeren in een beschrijvende diagnose. Daarnaast is het belangrijk er niet automatisch van uit te gaan dat patiënten met complex trauma wel complexe PTSS zullen hebben; bij veel patiëntengroepen is dit niet onderzocht of zijn de prevalenties relatief laag.
