12. Cognitieve therapie bij insomniastoornis

Samenvatting

Slaap is een basale levensbehoefte. Ongeveer 10 % van de volwassen Nederlandse bevolking lijdt aan een insomniastoornis. De definitie hiervan is minimaal drie nachten per week problemen met in- en/of doorslapen gedurende minimaal drie maanden. Klachten overdag, zoals moeheid, irritatie, concentratiegebrek en/of somberheid, zijn essentieel voor de diagnose. Onbehandelde insomniastoornis heeft een negatieve impact op de kwaliteit van leven en productiviteit, en is bovendien een risicofactor voor het ontwikkelen van psychische stoornissen, met name een depressie. Cognitieve gedragstherapie is de behandeling van eerste keus bij insomniastoornis. Uit onderzoek blijkt dat deze behandeling vooral positief effect heeft op het verkorten van de inslaaptijd en het verbeteren van de ervaren slaapkwaliteit. Bij 70–80 % van de patiënten verbetert de slaap. Deze effecten zijn vergelijkbaar met of beter dan die van slaapmedicatie. Ook bij patiënten met comorbide psychische stoornissen is de behandeling van insomnie aan te raden omdat het zowel positief effect heeft op het beloop van de behandeling van de psychische stoornis als directe positieve effecten op de slaap laat zien. Psycho-educatie over slaap, algemene slaaphygiënische adviezen, zelfregistratie met een slaapdagboek en cognitieve gedragstherapeutische interventies zijn de belangrijkste elementen van de behandeling. De behandeling richt zich op het herstellen van het automatische proces wat slaap in essentie is.
