Cognitief functioneren van prostaatkankerpatiënten die hormonale therapie ondergaan

Abstract

Prostaatkanker is een veel voorkomende vorm van kanker bij mannen. Jaarlijks worden erin Nederland meer dan 9000 nieuwe gevallen geregistreerd (Integrale Kanker Centra, 2010). Hormonale therapie is een belangrijke en veel toegepaste behandeling, naast radiotherapie (bestraling), operatie en chemotherapie. Voor patiënten met metastasen (uitzaaiingen) is hormonale therapie de eerste keuze van behandeling. Ook bij de behandeling van locaal uitgebreide prostaatkanker wordt hormonale therapie in toenemende mate toegepast. Hormonale therapie heeft bijwerkingen die een negatief effect hebben op de kwaliteit van leven. De meest voorkomende bijwerkingen zijn verlies van libido en erectiele disfunctie. Daarnaast kunnen, afhankelijk van het type hormonale therapie, onder andere opvliegers, gewichtsverandering, gewrichtsklachten en botontkalking voorkomen (Van den Heijkant et al., 2005). De laatste tijd is er toenemende aandacht voor de vraag of hormonale therapie zou kunnen leiden tot een verminderd cognitief functioneren van patiënten met prostaatkanker (Schilder et al., 2006). In dit artikel geven wij een overzicht van de huidige neuropsychologische kennis op dit gebied, beschrijven wij eigen onderzoek aan het Nederlands Kanker Instituut en bespreken wij mogelijke implicaties voor de klinische praktijk.
