Samenvatting

In H. 6 en in deel van dit boek hebt u kunnen lezen over diagnostiek en behandeling van chronische nierschade. De stijgende prevalentie van chronische nierschade noopt tot goed gestructureerde zorg om diagnostiek en behandeling op effectieve wijze vorm te geven. Dit hoofdstuk gaat in op organisatorische aspecten rond de zorg voor patiënten met chronische nierschade. Daarbij hanteren we de volgende uitgangspunten:
  • De zorg wordt waar mogelijk in de eerste lijn geleverd.
  • Alleen waar nodig vindt de zorg plaats in de tweede lijn.
  • Protocollaire zorg wordt gegeven door praktijkondersteuners.
  • Er wordt optimaal gebruikgemaakt van de deskundigheid van betrokken disciplines: apotheker en nefroloog.
