Abstract

The cause of symptomatic bradycardia in most patients is either sinus node disease or atrioventricular block. The aetiology is usually generalized disease of the conducting tissue and sensitive tests may reveal the coexistence of both defects in the same patient. In practice, however, it is uncommon to find this to a clinically significant degree: in sinus node disease it is not generally necessary to pace the ventricle and in heart block it is not generally necessary to pace the atrium. Therefore the logical choice of pacing mode to restore physiological rhythm is AAI or AAIR for sinus node disease and VDD for atrioventricular block.
