Zusammenfassung
Die Sonographie des Skrotalinhalts hat heute in der apparativen andrologischen Diagnostik einen festen Platz (Isidori u. Lenzi 2008). Die Skrotalorgane sind durch die Sonographie einer nebenwirkungsfreien, bildgebenden Diagnostik zugänglich. Normale Hoden und Nebenhoden weisen eine homogene Echostruktur des Parenchyms auf. Die palpatorische Hodenvolumenbestimmung kann durch Hydrozelen, verdickte Skrotalhaut, fibrosierte Nebenhoden oder insbesondere durch Lageanomalien erschwert sein. Hier trägt die Sonographie zur Objektivierung des Befundes bei. Unter Anwendung der Formel für das Rotationsellipsoid ist eine präzise und reproduzierbare Volumenbestimmung möglich (Behre et al. 1989) (◘ Abb. 6. 1a), die eine besondere Bedeutung gerade bei Therapieverlaufsstudien (z.B. Gonadotropinbehandlung bei hypogonadotropem Hypogonadismus) erlangt (Büchter et al. 1998). Eine Hydrozele erscheint als areflexiver Randsaum um den Hoden (◘ Abb. 6. 1b). Sie kann als Begleithydrozele nach Operationen, bei Hodentumoren oder chronischen und rezidivierenden Entzündungen der Testes und Nebenhoden auftreten.
