Abstract

Bipolar disorder in children remains a controversial topic in the field of child and adolescent psychiatry. Comorbidity with other mental illnesses, diagnostic criteria, and consequent treatment are among the issues debated within the international community. While DSM-V has allowed for increased diagnostic latitude, many practitioners find diagnosing children with bipolar disorder to be challenging. This has been attributed to multiple reasons, including the greater frequency of mixed episodes in children, the unreliability of child self-report, the fact that children frequently do not meet clear symptom duration criterion, and the difficulty in making clear distinctions between clinically significant symptoms and developmentally normative behaviors. Over the past decade, there has been an explosion of research on childhood bipolar disorder as researchers search for diagnostic clarity. A large number of review papers have been published that seek to consolidate information for the scientific community, laying the foundation for recent research papers investigating brain structure, brain function, and pharmacology. Multiple longitudinal, DSM-based studies have been conducted, with the most influential covered within this chapter. It is our hope that through detailed discussion of the historical context, etiology, symptom presentation, current best practices regarding treatment, and prognosis of childhood bipolar disorder, we will help researchers and clinicians better serve this unique population.
