Abstract
Due to the complex nature of intellectual disabilities, multiple professionals must work together to promote quality transition services and successful post-school outcomes. The composition of these teams may vary based on the complexity of a student’s disability or disabilities, the student’s age, and the context in which services are provided. Schools, community agencies, healthcare providers, and nonprofits can work together to provide quality supports and services for individuals, but there are innate challenges. This chapter explores how to achieve productive collaboration by incorporating specific strategies and developing necessary capacities for promoting and facilitating adult professional teaming.
