Part of the book series: Critical Issues in Psychiatry ((CIPS))

Abstract

According to the revised third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-III-R; American Psychiatric Association, 1987), obsessions are “recurrent and persistent ideas, thoughts, impulses, or images that are experienced... as intrusive and senseless” (p. 245). The individual recognizes the obsessions as a product of his or her own mind and attempts to ignore, suppress, or neutralize them with some ritualistic thought or action. The content of clinical obsessions differs little from the intrusive thoughts exhibited by nonobses-sionals, but the former provoke more anxiety and are less easily dismissed than the latter (Dent & Salkovskis, 1986; Rachman & De Silva, 1978; Salkovskis & Harrison, 1984). Compulsions are repetitive, stereotypical behaviors performed in response to an obsession. They are designed to reduce discomfort or prevent a feared event, although this intention may not always be apparent to an observer. Rituals are excessive and are recognized as unreasonable by the individual.
