Abstract
A structured survey featuring both open-ended and multiple-choice questions. Intended as a clinical tool, the instrument allows for the collection of demographic details, familial and medical histories, and information regarding sleep habits, schedules, and behaviors [1]. Three versions of the survey have been created: two designed for self-report (differing only in their mention of either male- or female-related developmental milestones), and one for the adolescent’s parent or guardian to complete.
