Samenvatting

Dit hoofdstuk gaat over atherosclerose van de extracraniële arteria carotis. Met een carotisstenose bedoelt men meestal een vernauwing in de carotisbifurcatie en/of in het begin van de arteria carotis interna. Patiënten met een atherosclerotische afwijking in de carotisbifurcatie kunnen asymptomatisch zijn of een variëteit aan neurologische symptomen vertonen. Hierbij wordt onderscheid gemaakt tussen een ‘transient ischaemic attack’ (TIA) en een ischemisch cerebrovasculair accident (CVA) of herseninfarct. Duplexechografie is de initiële vorm van diagnostiek, omdat dit onderzoek niet-invasief, risicoloos, relatief goedkoop en accuraat is om de ernst, locatie en uitbreiding van de stenose vast te stellen. Meestal gebruikt men een duplex in combinatie met een CTA indien een operatie lijkt aangewezen. Bij een carotisstenose dient secundaire preventie middels cardiovasculair risicomanagement (CVRM) gestart te worden. Daarnaast zal bij iedere symptomatische patiënt met (recente) symptomen gekozen worden voor een chirurgische (carotisendarteriëctomie) of endovasculaire revascularisatie (carotisstenting). Revascularisatie dient binnen twee weken na het ontstaan van een TIA of herseninfarct uitgevoerd te worden.
