Samenvatting

Cardiovasculaire aandoeningen komen vaak voor bij oudere personen en zijn in deze doelpopulatie de belangrijkste doodsoorzaak. Kransslagaderlijden en hartfalen zijn hierin de belangrijkste en meest courante aandoeningen die de fysiotherapeut/kinesitherapeut in de klinische praktijk zal tegenkomen en waarin de nood aan revalidatie zeer hoog is. Hoewel een verhoging in fysieke activiteit en revalidatie aanleiding geeft tot een hogere levensverwachting, lagere morbiditeit en/of lagere hospitalisatiefrequentie bij patiënten met kransslagaderlijden en/of hartfalen, dient er in hartrevalidatieprogramma’s voldoende aandacht te worden besteed aan specifieke noden en kenmerken van oudere personen met dergelijke aandoeningen. Deze doelpopulatie heeft namelijk regelmatig te maken met een scala van comorbiditeiten (inclusief de aanwezigheid van fragiliteit) en hun belastbaarheid is vaak verminderd. In dit hoofdstuk wordt uitvoerig toegelicht hoe revalidatie bij oudere personen met kransslagaderlijden en/of hartfalen dient te worden geïmplementeerd. Aan de hand een casus van een 87-jarige man met hartfalen en een pacemaker, en een voorgeschiedenis van kransslagaderlijden wordt de praktische implementatie van revalidatie toegelicht.
