Can variables from the transtheoretical model predict dietary change?

Abstract

The present study was designed to see whether variables from the transtheoretical model could account for changes in behavior, as opposed to changes of stage. A broadly representative sample (N = 356) completed measures of transtheoretical model constructs and dietary intake at baseline; dietary intake was measured at follow-up. One of the processes of change, self-reevaluation, predicted changes in behavior and significantly mediated the effect of past behavior on future behavior. It is rare that social cognitive variables mediate the effects of past behavior on future behavior meaning that self-reevaluation provides a potential avenue for promoting dietary change.

It is not immediately obvious why the stages of change have been so thoroughly investigated while the processes of change have been relatively neglected. One possible explanation is that measuring the processes of change requires at least thirty items, which may be perceived as off-putting to researchers working with people in field settings. Second, the stages of change measure offers a neat, intuitive and plausible means of classifying individuals, and judging by the pattern of citations of the transtheoretical model, it is something that appears to chime with colleagues who are faced with the realities and time pressures of treating patients in primary care settings (see Gatchel and Oordt 2003). It is perhaps therefore unsurprising that research into the stages of change seems to have usurped inquiry into the processes of change. One notable exception to this pattern of neglect is work that has begun to use the processes of change as an intervention tool in their own right (see Armitage 2008, 2009b; Armitage and Arden, in press).

Note that because decisional balance and self-efficacy are conceptualised as dependent variables within the transtheoretical model they should mediate any effects of the processes of change (independent variables) on behavior change. Decisional balance and self-efficacy were therefore entered on the final step of the analyses.

Note that another way of interpreting these analyses is that they provide evidence for causal relationships. Regressing Y at Time 2 on X at Time 1 can find a causal relationship between variables X and Y while simultaneously controlling for Y at Time 1. Any remaining variance that is explained in Y at Time 2 by X at Time 1 can be interpreted as showing that X causes Y (see Cohen et al. 2003).
