Samenvatting

Dit hoofdstuk concentreert zich op de diagnose late onset asthma: astma waarbij de diagnose pas gesteld wordt op volwassen leeftijd. Kenmerkend voor astma dat is ontstaan op volwassen leeftijd is dat de longfunctie al kort na het begin van de klachten verminderd is en vrij snel kan afnemen, waarbij patiënten vaak prednisonafhankelijk worden. Het gaat bij patiënten met ernstige late onset asthma om een sterk heterogene groep. Twee verschillende fenotypes van late onset asthma komen met name naar voren: het ene wordt gekenmerkt door persisterende eosinofiele inflammatie van de luchtwegen, en het andere door adipositas als comorbiditeit. De differentiaaldiagnose wordt besproken. In principe verschilt de behandeling van astma bij volwassenen niet van die bij kinderen.
