10. Bekkenbodemdisfuncties

Samenvatting

Ouderen hebben veelal te maken met complexe en multipele problematiek: chronische aandoeningen als diabetes, COPD en dementie, acute problemen bijvoorbeeld na een val of CVA, maar ook met bekkenbodemdisfuncties als urine-of ontlastingsverlies, recidiverende urineweginfecties of obstipatie. Bekkenbodemdisfuncties zijn voor geriatriefysiotherapeuten/geriatriekinesitherapeuten vaak nog onbekend terrein, terwijl ze een duidelijke relatie hebben met mobiliseren, valrisico en cognitieve achteruitgang. De prevalentie is hoog: urineverlies wordt gemeld door bijna 70 % van de ouderen, ontlastingsverlies door ruim 40 %. Ouderen met incontinentie hebben door schaamte de neiging minder te bewegen, waardoor het risico ontstaat dat men in een negatieve spiraal terechtkomt die leidt tot passiviteit en sedentair gedrag en bovendien tot minder bereidheid te trainen. Bekkenbodemdisfuncties zijn een negatieve prognostische factor voor herstel. Derhalve is het bij de behandeling van ouderen belangrijk om het functioneren van de bekkenbodem mee te nemen in de diagnostiek en de therapie.
