Samenvatting

Aan de ontwikkeling van een antisociale persoonlijkheidsstoornis (ASPS) ligt vaak vroegkinderlijke, complexe traumatisering ten grondslag. Veel cliënten met ASPS zijn in hun kindertijd slachtoffer of getuige geweest van mishandeling, agressie, (be)dreiging of seksueel misbruik. Daarnaast hebben cliënten met ASPS door hun impulsieve levensstijl een verhoogde kans om later in hun leven trauma’s mee te maken. In dit hoofdstuk zetten we uiteen welke keuzes kunnen worden gemaakt in de behandeling van psychotrauma bij ASPS en hoe de behandeling het beste kan worden opgezet en uitgevoerd, waarbij er in het bijzonder aandacht is voor het behandelen van een posttraumatische stressstoornis (PTSS) bij agressieproblematiek. Dilemma’s die behandelaren kunnen weerhouden van traumabehandeling bij ASPS zijn onder andere bezorgdheid over toename van agressie, geen goede werkrelatie kunnen opbouwen, de schijnbare afwezigheid van spanning, angst voor terugval in middelengebruik en moeite de eigen boosheid of verontwaardiging over het gedrag van de cliënt te hanteren. We illustreren hoe deze dilemma’s kunnen worden omgezet naar behandelmogelijkheden.
