Behandeling van obstructieve slaapapneu

Samenvatting

Het obstructieveslaapapneusyndroom (OSAS) is een veelvoorkomende aandoening, vaak gepaard gaande met hinderlijke en soms invaliderende symptomen. Daarnaast is er een bewezen risico op cardiovasculaire aandoeningen, bij matig en zeker bij ernstig OSAS. Het doel van de behandeling is het wegnemen van de symptomen en de risico's op hart- en vaatziekten.
Indien er sprake is van een symptomatisch obstructieve slaapapneu, dient als eerste gekeken te worden naar de noodzaak van conservatieve maatregelen. In slechts een gering percentage blijkt dit afdoende en moet nog een aanvullende behandeling worden gegeven. Bij rugligging-gebonden OSAS kan houdingstherapie effectief zijn. Wanneer de obstructie op het niveau van de velofarynx aanwezig is, kan een kno-ingreep bij minder ernstige vormen van OSAS uitkomst bieden. Anderen hebben meer baat bij een mandibulair repositieapparaat (MRA) vooral bij obstructie op tongbasisniveau. Het merendeel van de patiënten, veelal met een ernstig OSAS, wordt behandeld met CPAP. Bij patiënten met OSAS en ernstig overgewicht wordt steeds vaker overgegaan tot bariatrische chirurgie.
Het is van groot belang dat er een multidisciplinaire benadering is van deze aandoening, zodat de patiënt op maat geholpen kan worden en dat er alternatieve behandelingen mogelijk zijn bij het falen van de initiële therapie.
