12. Behandeling met psychofarmaca bij cliënten met antisociaal gedrag of een antisociale persoonlijkheidsstoornis

Samenvatting

Medicatiestrategieën kunnen aanvullend de klachten en symptomen van mensen die antisociaal gedrag vertonen beïnvloeden. Vooral reactieve of impulsieve agressie is een goede indicatie om psychofarmaca in te zetten in de behandeling. In dit hoofdstuk geven we adviezen als aanvulling op bestaande richtlijnen waarin de problematiek van antisociaal gedrag in de reguliere GGZ niet volledig wordt beschreven. We lichten de behandeling van comorbide stoornissen als PTSS, ADHD, stoornissen in gebruik van middelen, angst-, stemmings-, en psychotische stoornissen uit. Verder staan we stil bij de vraag hoe om te gaan met een lage motivatie om medicijnen te nemen voor antisociaal gedrag. Daarnaast dient er aandacht te zijn voor somatische factoren die antisociaal gedrag uitlokken, in stand houden of er het gevolg van zijn. In de praktijk dient de psychiater rekening te houden met mogelijke ongewenste risico’s van psychofarmaca, zoals een toename van delictgedrag of misbruik van middelen. Betrekken van het systeem van de cliënt is belangrijk bij het inschatten van dergelijke risico’s.
