4. Behandeling met gedragstherapie of toegepaste gedragsanalyse bij kinderen van één tot zes jaar

Samenvatting

De empirische evidentie voor de behandeling van ARFID bij jonge kinderen is vooral afkomstig uit de toegepaste gedragsanalyse. Interventies worden gekozen op basis van uit analyse verkregen gegevens, wat uiteindelijk leidt tot een idiosyncratisch behandelpakket. De analyse kenmerkt zich door het opsporen van respondente en operante conditioneringspatronen, zowel vanuit de kant van het kind, als van die van de ouders en de omgeving. Die patronen dragen bij aan de bestendiging van het problematische eetgedrag, wat zich veelal kenmerkt door angst. Uiteraard wordt rekening gehouden met het etiologische ARFID-profiel waarmee het kind belast is.
In dit hoofdstuk wordt getracht de lezer op een methodische wijze mee te nemen in de te zetten stappen om tot een positieve verandering van het eetgedrag te komen en de eetangst te doen afnemen. De therapeut krijgt een arsenaal van gedragstherapeutische technieken aangereikt, die ingezet kunnen worden om de respondente en operante conditioneringspatronen het hoofd te bieden.
Daarnaast wordt de rol van de ouders in dit proces belicht en de manier waarop de therapeut samen met hen het eetgedrag kan normaliseren.
