20. Bariatrische en metabole chirurgie

Samenvatting

(Morbide) obesitas is wereldwijd een ernstig en explosief groeiend gezondheidsprobleem. Conservatieve behandelingen, zoals een dieet en gedragstherapie, zijn vaak niet succesvol op de lange termijn. Bariatrische chirurgie is de enige therapie die een significant en langdurig gewichtsverlies kan bewerkstelligen bij morbide obesitas. Laparoscopic adjustable gastric banding is vanwege complicaties en matige resultaten op lange termijn inmiddels obsoleet, maar er zijn nog wel veel patiënten de deze LABG-operatie hebben ondergaan. Roux-en-Y gastric bypass (RYGB) ofwel maagomleiding is inmiddels de gouden standaard. Andere gangbare operatietechnieken zijn gastric sleeve-resectie en mini gastric bypass. Bariatrische chirurgie heeft naast gewichtsverlies een gunstig effect op comorbiditeit, in het bijzonder op diabetes mellitus. Zodoende wordt de term metabole chirurgie steeds vaker gebruikt. Metabole chirurgie dient onderdeel te zijn van anti-diabetesbehandelingen voor patiënten met een combinatie van DM-II en obesitas. Inmiddels zijn er ook nieuwe technieken ontwikkeld. Enkele voorbeelden hiervan zijn transorale gastroplastiek (ToGa), endoscopische botuline-injectie, maagstimulatie door middel van ‘pacing’, en de duodenal-jejunal bypass sleeve (EndoBarrier Gastrointestinal Liner).
