Anxiety Sensitivity Moderates Relations Among Tobacco Smoking, Panic Attack Symptoms, and Bodily Complaints in Adolescents

Abstract

The present study evaluated associations among smoking and anxiety sensitivity (fear of anxiety and anxiety related sensations) in predicting panic attack symptomatology, somatic complaints, and depressive symptomatology in a community sample of adolescents (ages 12 to 17 years; N=206). As predicted, the combination of high levels of anxiety sensitivity and being a current smoker predicted panic symptomatology and somatic complaints, but not depressive symptomatology. These findings suggest anxiety sensitivity may moderate the relation between smoking and prototypical panic psychopathology variables (panic attacks and somatic complaints) even after controlling for gender and negative affectivity, and that these associations are specific to panic-relevant processes. The primary implication of the study findings is that there may be segments of the adolescent population who are at relatively greater risk for panic-related problems by virtue of individual differences in AS and smoking status.

It should be noted that the main and interactive effects of lifetime smoking (i.e., responses to the question, “have you ever tried smoking, even one or two puffs? [yes/no]) also were examined. Thirty-nine percent (n=81) of the sample responded affirmatively to this question. Even when using this broad-based index of tobacco exposure, the pattern of findings does not change in nature or magnitude.

Unfortunately, no data were collected on the 11 adolescents who declined to participate in the present study after informed assent forms were administered. Without exception, adolescents in this group decided they did not have the time/interest to complete the questionnaire battery. Although the absence of differences between this sub-group and the rest of the sample cannot be empirically demonstrated, in light of the fact that the large majority of potential participants did elect to take part in the study (i.e., 97.63%), the authors are reasonably confident that no significant differences exist between the 206 adolescents who did participate and the 11 who did not.

In light of the fact that smoking co-occurs with other types of substance use, and alcohol use in particular (Amos et al., 2004), analyses also were ran with current alcohol use covaried at level one of the model. (Responses to the question, “During the past 30 days, on how many days did you have at least one drink of alcohol? [with the exception of sips taken for religious purposes]?” were coded dichotomously; 0 = 0 days; 1 = 1 or more days). Analyses indicated the pattern and magnitude of the findings is unchanged when alcohol use is covaried at step one of the model. These findings are footnoted rather than included in the narrative because polysubstance use may be a defining feature of high risk populations (Grant et al., 2004). Thus, to covary for the associations of another drug while exploring smoking-panic associations may alter how the model looks in its purest form (see Miller & Chapman, 2001, for expanded discussion of this issue).
