17. Antistolling en tandheelkundige ingrepen

Abstract

Sinds 2006 is er een overeenkomst met de overkoepelende Federatie Nederlandse Trombosediensten, dat bij bloedige tandheelkundige ingrepen, bij patiënten die antistolling gebruiken met cumarinederivaten, de regionale diensten in overleg met de betrokken tandarts en patiënt de antistolling regelen. De controle van de International Normalized Ratio (INR) en de herstart van cumarinederivaten blijven volgens deze afspraak in handen van de trombosediensten (Abraham, 2004; Levi en Frank, 2006). Een brief in 2008 van de regionale trombosedienst Leiden aan de tandartsen in de regio over het continueren van antistolling, maakte niet alleen een discussie los, maar veroorzaakte ook veel onzekerheid met betrekking tot het handelen in de praktijk (afb. 17.1). Om bestaande adviezen terzijde te kunnen schuiven en nieuwe aanbevelingen en behandelinstructies uit te vaardigen, dient men zich te baseren op ‘evidence-based medicine’, berustend op prospectief, zo mogelijk dubbelblind gerandomiseerd onderzoek. Dit type onderzoek ontbreekt. De literatuur over dit onderwerp berust voornamelijk op ‘me too’-producties of op samenvattingen van onderzoek van anderen (Todd, 2003; Beirne, 2005; Van Diermen et al., 2008; Jiménez et al., 2008; Khamashta-Ledezma, 2008). Door het ontbreken van goed onderzoek en omdat men zich in de literatuur zonder onderbouwing baseert op ‘de mening van deskundigen’, moet kritisch worden gekeken naar alles wat als vaststaand wordt aangenomen. In het buitenland wordt veelal warfarine (Coumadin®) voorgeschreven. In Nederland en België wordt echter meestal gewerkt met acenocoumarol (Sintrom[Mitis]®) en fenprocoumon (Marcoumar®). De werking van deze drie producten komt weliswaar overeen, maar de werkingstijd verschilt (tabel 17.1). De beschikbare literatuur is voor het merendeel op warfarine geënt en is daarmee niet direct te transformeren naar de Nederlandse situatie. In dit hoofdstuk wordt getracht op grond van de beschikbare literatuur een leidraad voor de dagelijkse praktijk te ontwikkelen.

Vrijwel al het onderzoek met betrekking tot INR en tandheelkunde betreft extracties. Voor parodontale chirurgie en implantologie wordt voorlopig gesteld dat een INR van 3.5 te hoog is! Kritiek op deze uitspraak wordt ondervangen door de worden: ‘We zeggen niet dat de INR 3.5 moet zijn, het mag lager!’ Indien men veel patiënten op antistolling heeft, of de Regionale Trombosedienst is niet in staat om kort voor de ingreep een INR te bepalen, wordt in de literatuur aangeraden meetapparatuur voor de INR voor de praktijkvoering aan te schaffen. Niet om de patiënt in te stellen, maar voor controle bij een bloedige ingreep.
