8. Amelogenesis imperfecta

Samenvatting

Amelogenesis Imperfecta is een aangeboren afwijking die de structuur en de klinische verschijning van het tandglazuur aantast en mogelijk geassocieerd is met morfologische en biochemische veranderingen elders in het lichaam. De prevalentie varieert van 1 op 700 tot 1 op 14.000. Amelogenesis imperfecta komt in verschillende verschijningsvormen voor: hypoplastisch glazuur, hypocalcificatie van het glazuur en hypomaturatie van het tandglazuur. Combinaties van deze vormen zijn ook mogelijk. Milde verschijningsvormen, met een glad egaal, maar verkleurd oppervlak worden vaak alleen vanwege sociale/esthetische redenen behandeld. Ernstiger vormen waarbij de anatomie van de tand approximaal is aangetast (contactpunten ontbreken) beïnvloeden ook de gezondheid van het parodontium en verdienen wel degelijk een tandheelkundige interventie. Bij occlusaal verlies van de anatomie dreigt passieve eruptie van het gebitselement, wat zeker voorkomen dient te worden. Tijdig behandelen is de boodschap om onnodige kosten in de toekomst te voorkomen en een eventueel orthodontische behandeling te vereenvoudigen en/of de duur ervan te bekorten.
