Samenvatting

De menselijke waarneming is neuraal geordend naar modaliteit (horen, zien, voelen) en binnen iedere modaliteit kan men weer verschillende modulen onderscheiden, bijvoorbeeld zien van beweging, van kleur, van gezichten. Een agnosie is een stoornis van de herkenning bij intacte primaire sensorische functie. Naar analogie van de indeling van de apraxieën kunnen ook bij de agnosieën twee uitersten onderscheiden worden: bij de associatieve (conceptuele) agnosie wordt de betekenis van de waarneming niet begrepen, bij de perceptuele agnosie gaat het om elementaire sensorische aspecten (vorm, kleur, toonhoogte, glad versus ruw). Los hiervan kan men de agnosieën indelen naar modaliteit: visuele (vormen, kleuren, gezichten), akoestische (spraakklanken, geluiden van het dagelijks leven, muziek) en tactiele agnosie (vorm, oppervlak). Daarnaast bestaan complexe of moeilijk in te delen vormen van agnosie, zoals de somato-agnosie (stoornissen van lichaamsschema), spatiële agnosie (ruimtelijke desoriëntatie) en simultaan-agnosie (geen verband zien tussen elementen). Agnosieën kunnen voor de patiënt onaangename gevolgen hebben: hij voelt zich onzeker in verkeer of supermarkt, is angstig of terughoudend in sociale contacten (stoornissen van gezichtsherkenning), schrikt van onbelangrijke dagelijkse geluiden, verdwaalt bij het uitlaten van de hond, snapt een cartoon in de krant niet.
