Afwezige pulsaties aan de pols na een supracondylaire humerusfractuur

Samenvatting

In dit artikel wordt een casus beschreven waarbij er geen pulsaties in de arteria radialis en ulnaris zijn na een gedislokeerde supracondylaire humerusfractuur. Er bestaat nog geen consensus over het beleid bij deze zogenoemde pink pulseless hand. In dit artikel wordt duidelijk dat er in de literatuur wel degelijk handvatten zijn die richting aan dit beleid kunnen geven. Wanneer er na repositie en interne fixatie geen perifere pulsaties zijn, bevestigd met duplexechografie, is er vanuit de literatuur onderbouwing om over te gaan tot exploratie van de a. brachialis. De ingreep is zeer succesvol met goede resultaten op lange termijn, waarbij de patiënt zijn dominant aanvoerende arterie behoudt.
