11a Addendum: het patellofemorale pijnsyndroom

Samenvatting

De term ‘patellofemorale pijnsyndroom’ is geen diagnose maar een symptoombeschrijving: de term geeft aan dat sprake is van kniepijn waar het patellofemorale gewricht bij betrokken is. Wát er exact mis is met het patellofemorale gewricht, is dus niet duidelijk. Vaak wordt aangenomen dat de pijn veroorzaakt wordt door het verkeerd sporen van de patella in de patellofemorale groeve. Harde bewijzen hiervoor ontbreken echter. De term wordt dus eigenlijk gebruikt voor die gevallen van anterieure kniepijn waarbij geen duidelijke oorzaak voor het probleem wordt gevonden. De ‘diagnose’ moet met enige terughoudendheid worden gesteld; als men tegen een patiënt met anterieure kniepijn zegt dat hij of zij lijdt aan een patellofemoraal pijnsyndroom, dan is dat dus eigenlijk: ‘U hebt anterieure kniepijn en ik weet niet waardoor het wordt veroorzaakt.’

Malalignement: eigenlijk mal-alignement ofwel ‘slecht uitgelijnde’ patella: door een abnormale anatomische positie glijdt de patella niet perfect door de femorale groeve tijdens flexie en extensie.
