Acute buik, niet-gynaecologische ontstekingen

Samenvatting

Het onderscheid tussen buikpijn die geen chirurgische interventie behoeft en een acute buik is niet altijd gemakkelijk te maken. Voor de extramuraal werkende arts is de belangrijkste uitdaging om met behulp van ogen, oren en handen te beoordelen of de patiënt al dan niet specialistische behandeling nodig heeft. Soms kan beperkt aanvullend onderzoek hem hierbij helpen. In dit hoofdstuk bespreken we alleen de infectieuze oorza ken van de acute buik: appendicitis, cholecystitis, pancreatitis, diverticulitis, peritonitis en abces.

Het missen van een acute buik of het achteraf ten onrechte instellen van een afwachtend beleid kan verstrekkende gevolgen hebben. Bij iedere patiënt met buikpijn zal de huisarts zich dan ook afvragen of er sprake is van buikpijn die chirurgische beoordeling noodzakelijk maakt.
