8 Overzicht van onderzoek en behandeling van arm- en handvaardigheden bij kinderen met een unilaterale cerebrale parese

Samenvatting

Kinderen met een cerebrale parese hebben een beschadiging van het centrale zenuwstelsel, die leidt tot diverse secundaire problemen op het gebied van, motoriek, sensoriek, communicatie, cognitie en emotie. Er zijn vele ontwikkelingen op het terrein van diagnostiek, classificatie, onderzoek en behandeling bij kinderen met CP. Bij het therapeutische beleid bij kinderen met CP wordt tegenwoordig meer uitgegaan van de mogelijkheden van het kind en uit onderzoek is gebleken dat de kracht in de handen en armen bij kinderen met een unilaterale CP aan beide zijden verstoord is. In dit hoofdstuk worden de uitgangspunten voor de klinimetrie met betrekking tot de arm- en handvaardigheden bij kinderen met CP uitgewerkt. Daarbij worden tests besproken die gebruikt worden bij het bepalen van de hulpvraag en de doelstelling van de therapie en bij het vaststellen van het participatie-, activiteiten- en functieniveau uit de ICF-CY. Voor de behandeling van kinderen met CP zijn een aantal mogelijkheden, zoals krachttraining en oefening van de precisie (zowel in de aangedane hand als in de voorkeurshand). De beïnvloeding van de afwijkende spiertonus is tegenwoordig omstreden, evenals het gebruik van botulinetoxine-injecties. In elk geval is het belangrijk zoveel mogelijk taakspecifiek te oefenen.
